Full Name

Enter full name of the principal investigator, e.g. John Smith.

User Name (Required)

Enter the principal investigator's user name - initial of first name followed by full last name (e.g. jsmith). No spaces or special characters. Usernames and passwords are case sensitive, make sure the caps lock key is not enabled. This is the name used to log in.

Organization (Required)

Enter the name of your organization.

Position (Required)

Enter the name of your position or title.

Telephone Number (Required)

Enter your 10-digit telephone number.

Fax Number

Enter your 10-digit fax number.

Address (Required)

Enter your full street address including city, state and ZIP code.

Mailing Address (Required)

Enter a mailing Address

E-mail (Required)

Enter the email address of the principal investigator. This is necessary in case the password is lost. We respect your privacy, and will not give the address away to any third parties or expose it anywhere.

A password will be generated and e-mailed to you to complete the registration process.
